Rehabilitation services for individuals with post-COVID-19 issues


West Park’s Post-COVID Outpatient Service is for individuals who have on-going or re-occurring symptoms of COVID-19 impacting their functional activities and are at least 4 weeks post infection. Community patients will be assessed by our Physician and participate in a rehab program. Our interprofessional team includes a Physiatrist, Registered Nurse, Physiotherapist, and Occupational Therapist. Consultation with other health disciplines may be facilitated as needed (e.g. Speech & Language Pathologist, Social Worker). Referrals to other medical specialties (i.e. Respirologist, Geriatrician) may be made, as required.

Post-COVID treatment focuses on helping patients develop the tools/skills to manage their symptoms and support a return to meaningful daily activities.  Treatment may include supervised exercise programming, education sessions focusing on energy conservation, mindfulness, memory compensation strategies, breathing strategies and sleep hygieneeurological Assessment and Follow-up Clinic.

Type: Wellness & Therapy

Funding: The cost of the service is covered for patients with a valid Ontario Health Insurance Plan (OHIP) card issued by the Ministry of Health and Long-Term Care.

Eligibility: Post-COVID Outpatient Rehabilitation

Referral requirements:

The service is open to adults (18 years of age and older) and be at least 4 weeks post onset of COVID-19 infection. Referral is through family physician or nurse practitioner.
