Description:
The Gage Transition to Independent Living (GTIL) offers a community setting in midtown Toronto for people with disabilities to learn and practice independent living skills. Clients are selected for either a six or 12-month ‘transitional’ program. Clients are provided with a one-bedroom apartment and access to on-site attendant services (24-hour) and health and life skills education.
The GTIL program offers a self-directed learning environment with practical learning in a variety of topics including:
- directing attendant services;
- buying groceries and planning meals;
- budgeting and money management;
- accessing community services, funding and services;
- managing one's health;
- applying for permanent housing.
Transitional Living
The cost of the service is covered for clients with a valid Ontario Health Insurance Plan (OHIP) card issued by the Ministry of Health and Long-Term Care. See "additional Information" for more detail on other costs.
Eligibility: Individuals 18 years of age and older who require attendant services and want to learn the skills necessary to live independently.
- Age 18 and older;
- Has a physical disability and requires attendant services for personal care;
- Is medically stable (needs must be met by family doctor, local health services including LHIN (formerly CCAC) and not at risk for frequent hospitalization);
- Has a desire to learn independent living skills;
- Has the cognitive ability to learn and manage independent living skills especially directing own care, health and financial management;
- Able to participate in ongoing program education sessions;
- Able to return to the community with established discharge supports.
Please note: GTIL is unable to support individuals with developmental disability, significant cognitive or mental health needs, or health needs that cannot be met through community services.

Applicants must be eligible for rent subsidy (either on ODSP or other income). Cost of attendant services and education at GTIL is covered by the Ministry of Health. Clients are responsible for furnishing apartment, subsidized rent and regular living expenses (phone, cable etc.)
