West Park's privacy practices and policies follow the requirements of the provincial government's Personal Health Information Protection Act (PHIPA). Our practices are also consistent with those of other hospitals across Ontario and are based on the 10 guiding principles outlined in the Canadian Standards Association (CSA) Model Code for the Protection of Personal Information; otherwise known as the "CSA Code."
Accountability
We are responsible for the safe-keeping of your personal health information and have a Chief Privacy Officer who is accountable for the centre's compliance with provincial privacy legislation.

We will explain to you the purposes for collecting personal health information and how that information will be used.

We will interpret your request for health care services as implying consent for the collection and use of your personal health information.

We will only ask you for information that is required for the purposes identified.

We will make reasonable efforts to ensure that your personal health information is accurate, complete and up to date.

We will take appropriate measures to protect your personal health information.
Openness
We will share with you the privacy policies and practices developed to protect your personal health information.
Individual access
We will allow you to access your own personal health information
Challenging compliance
We will address challenges relating to our privacy practices, and have a procedure to address your concerns.
