Complex Continuing Care
Ambulatory Care & Outpatient Services
Out-patient clinics and services for people
who live in the community
People with complex and ongoing health
problems need help from time to time to maintain or improve
their health and quality of life. West Park Healthcare Centre's
Ambulatory Care Service provides on-going care and treatment
after discharge and trains family members and caregivers to
provide care in the home.
The service is divided into three

northwest Toronto have priority; in some cases,
there are waiting lists

be booked in advance and all patients must
have a valid Ontario health card issued by
the provincial Ministry of Health and Long
Term Care

Amputee Clinics

Assistive Devices
Program (ADP) authorized mobility aids (except wheel chairs)

Clinic
