Acquired Brain Injury Behaviour Services - Community
Outreach
Assisting individuals who have experienced
an acquired brain injury to live more productive, active
and meaningful lives in the community
West Park Healthcare Centre's
Acquired Brain Injury Community Outreach Service provides
rehabilitation assistance for clients with challenging
behaviours as a result of an acquired brain injury (ABI)
who require therapy and support in order to live safely
in community settings, including their own homes, specializing
housing or hospitals.

The service provides in-patient
rehabilitation assistance for patients with
challenging behaviours and an acquired brain
injury who require behavioural rehab in order
to live in community settings

A skilled and dedicated
team of professionals focus on:
o Activities of daily living
o Skill deficits
o Participation in everyday activities, use
of compensatory strategies
o Aggression, anger and behavioural excesses
o Emotional liability

Our team includes behaviour
therapists, a social worker, and a psychologist.
We work with a client and his or her support
network to implement a behavioural change
plan that is developed collaboratively after
an assessment in the client's own environment

The team will help family
members, agency and hospital staff members
to develop strategies to meet a client's particular
needs

The advantage of West Park's
collaborative approach is that the service
is primarily delivered to individuals by people
known to them, in their own environments where
they live, work and learn

Clients of West Park's ABIBS
are adults (18 to 65 years of age) with an acquired
brain injury as the result of a trauma or a disease
process that has resulted in challenging behaviours.

Clients should:
o be medically stable
o be under the care of a physician
o not require immediate hospitalization for medical
or psychiatric reasons
o not be facing criminal charges
