Bertha Pesnick "The staff is very fine.f It's sort of a family affair."

A Serious Fall

Bertha could not live alone after her fall. West Park Long Term Care Centre has become home. The beautiful grounds are her garden.

Our Commitment to Transparency and Accountability

How we operate

West Park Healthcare Centre's mission is to help residents live the fullest lives possible. A big part of how we measure our success is by how residents and families feel about the care and services we provide.

We encourage feedback from both, and are committed to transparency and meeting both provincial and national standards of care. Our November 2004 voluntary accreditation survey by the Canadian Council on Health Services Accreditation (CCHSA) is a testament to that commitment.

West Park's long-term care standards compliance record has always been a matter of public record. After each annual Ministry of Health and Long-Term Care evaluation, we post our compliance reviews in the facility. We work not only with the ministry, but also with our residents, families and staff to ensure that any findings are resolved in a timely manner. We treat complaints and concerns as opportunities to improve the care and service we provide to residents, and strive to communicate during the resolution process in an open and honest manner.

Our current status

For the compliance period July, 2004 to June 2005, West Park Healthcare Centre had few major compliance issues. All of the compliance standards identified by compliance advisors during the period have been addressed.

Three new issues have been identified. In each of these cases ongoing attention has been planned for and is being implemented.

• Documentation of fully developed residents’ nutritional plans of care is being audited monthly;
• there are continuous quality improvement meetings being held monthly to discuss trends and issues and a two-year environmental plan has been developed to ensure that environmental standards are maintained; and
• water temperature is being monitored daily.

We are confident that that our compliance plans have addressed the concerns that have been raised this year. If you have any question about our quality initiatives, you are encouraged to discuss them at any time with Long Term Care Centre Executive Director, Trish Talabis.
Ms. Talabis can be reached at 416-246-5232 or by email at ttalabis@extendicare.com .
