Applying for Admission

Who is eligible?

Anyone with an OHIP card issued by the provinicial Ministry of Health and Long-Term Care who has a physician referral and is approved by the York Community Care Access Centre is eligible for admission.

How do you apply?

Admissions to long-term care services throughout the province of Ontario are regulated by Community Care Access Centres. For information on and admission to West Park's services, please contact:
