| Qualifications | - University degree in Psychiatric Nursing, Social Work, Occupational Therapy or other mental health discipline
- Masters preferred
- Registration with or eligibility for registration with an appropriate regulatory college
- Minimum of 5 years clinical experience with geriatric clients and their families in a clinical community setting
- Must have a Valid Canadian Drivers License and access to a reliable vehicle
- Excellent interpersonal, communication skills (written and verbal)
- Excellent interviewing and assessment skills
- Strong knowledge of community resources and demonstrated ability to establish linkages with community service providers
- Demonstrated teaching skills with families and professional service providers
- Excellent organizational and problem solving skills
- Valid drivers license and access to a reliable vehicle
- Evidence of satisfactory attendance and punctuality
- Experience in adult education (organizing / presenting)
- Computer skills
- Evidence of good performance history and attendance record
- Should be aware of conditions and measures that may affect the safety of patients, residents, students, employees, volunteers, physicians and visitors

| Responsibilities | - Provide comprehensive assessment, consultation and education to geriatric/psychogeriatric clients and their caregivers in a clinical and/or community setting
- Participate in case conferences with Clinical Consultant (Psychiatrist)
- Prepare written reports outlining intervention strategies
- Establish partnerships and liaisons with appropriate community agencies, organizations and committees whose objectives correspond to those of the Service
- Ensure timely and accurate clinical documentation and completion of workload
- Make appropriate referrals and provide guidance to clients and families as required
- Participate in program planning and evaluation
- Participate in the development and provision of educational materials and programs for clients, families and community service providers
