Qualifications
  • Masters degree in Nursing
  • Post Masters Acute Care Nurse Practitioner (ACNP) certificate or Primary Health Care certificate from an accredited Canadian nurse practitioner program. In good standing with the College of Nurses of Ontario
  • Three (3) Years Nurse Practitioner experience in an intensive care setting
  • Registered in the Extended Class or in the process of becoming registered
  • Expertise with clinical management of the respiratory patient population an asset
  • Demonstrated ability in best practice and /or protocol development and implementation; clinical evaluation; and clinical development.
  • Excellent interpersonal, communication, facilitation, negotiation and conflict management skills.
  • Comprehensive knowledge of clinical health issues associated with the specific clinical population.
  • Demonstrated ability to provide clinical leadership and mentoring to staff
  • Evidence of good performance history and attendance record
  • Ability to work in a manner that exemplifies the core values of West Park: Excellence, Respect, Trust, Collaboration, and Accountability
  • Should be aware of conditions and measures that may affect the safety of patients, residents, students, employees, volunteers, physicians and visitors



Responsibilities

    PRACTICE:
  • Provides primary care including episodic physical examinations of patients, full physical exams on new patients and modified diagnostic examinations based on specific symptoms and/or complaints as per the requirements to achieve a medical diagnosis. Some clinical services may require home visits
  • Based on medical directives prescribes treatment for clinical problems including the initiation/renewal/discontinuation of medications, laboratory and diagnostic testing, and referral for specialized consultation
  • Assists in review and management for psychosocial and family issues
  • Provides support for management of advanced directives


    PLANNING & EVALUATION
  • Assists staff in the development of plans of care
  • Assists the service manager in the identification of care needs
  • Identifies staff development and education needs
  • Assists in the evaluation of clinical practice


    LEADERSHIP
  • Assists in the design, methodology, database management and preparing of research reports and/ or clinical evaluation
  • Monitors effectiveness of treatment plans and recommends modifications accordingly
  • Guides the provision of care
  • Provides practice leadership in program planning
  • Assists in the review, revision, application and implementation of practice protocols and practice guidelines


    DEVELOPMENT
  • Provides assessment of staff learning needs
  • Plans and implements effective educational programs
  • Provide support for clinical development related to primary health needs
  • Shares knowledge at clinical and research levels
