| Qualifications | - Bachelors degree in a clinical discipline, preferably BScN
- Registration with appropriate regulatory college or eligibility for same
- Minimum of 4 years clinical professional background
- Previous experience with musculoskeletal, medical, surgical and orthopaedic required
- Clinical knowledge relating to MSK conditions
- Previous experience working with WSIB clients, WSIB case managers and legal representatives
- Previous experience working with 3rd party payers
- Able to multi-task and work with multi-disciplinary team in a fast-paced environment
- Demonstrated clinical assessment skills including the ability to assess clients from pre-admission to post discharge
- Knowledge of external agencies and resources
- Demonstrated ability to partner and work effectively with all levels of staff, the transdisciplinary team, physicians, clients, families and external agencies
- Demonstrated ability to deal with complex mental health and behavioural issues
- Ability to maintain effectiveness in varying environments and with different tasks, responsibilities and people
- Demonstrated knowledge and utilization of leading practices in integrated client focused care
- Demonstrated organizational, problem solving, interpersonal, coaching, negotiation and facilitation skills and critical thinking skills
- Strong organizational skills to work independently in fast paced environment
- Ability to work a flexible schedule
- Strong Windows-based computer skills e.g. Microsoft Office Suite

| Responsibilities | - Assess clients for appropriate admission to service and liaise with physicians and team
- Manage wait-list and occupancy rates for the service
- Liaise with referring agencies and provide ongoing support/education during the pre-admission phase
- Initiate goals of care plan with clients
- Initiate transdisciplinary assessment
- Co-ordinate the care of clients from pre-admission to post-discharge while meeting length of stay and occupancy rate targets
- Refer and coordinate consultant / discipline involvement as needed
- Serve as the co-ordinating contact between clients, family, team members and referring agencies
- Support collection of clinical utilization data in collaboration with Service Manager
- Provide resource clinical leadership for the transdisciplinary team
- Provide leadership in program evaluation and program development
- Participate in external committees
- Partner with Service Manager to ensure clinical service goals are addressed
- Ensure that care is provided in a client-focused approach with an emphasis on cultural care
- Ensure that clinical care issues/concerns are addressed in a timely and effective manner
- Ensure timely and accurate completion of workload & clinical documentation.
